On date and time listed above, Licensing Program Analyst (LPA), Sumayya Habeebulla and Licensing Program Manager (LPM) Carlos Martinez conducted an unannounced Case Management inspection for an increase in capacity. Upon arrival, LPA and LPM met with Facility Director Maria Lara.

The Licensee is requesting to increase capacity of Preschool children in Preschool Classrooms A & B. A fire clearance was granted for an increase of capacity on 08/22/2024. All indoor and outdoor activity space utilized for the children was inspected today and all new additional indoor activity space was measured. The total indoor activity space for Room B measured at 395.64 sq ft, and Room A measured at 136.32 sq ft, which is sufficient to accommodate the requested capacity of 39 children for all three classrooms – A, B, & C. A total of 3 sinks and 3 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 39 children.

· Indoor activity space was complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children.
· Classrooms and equipment are free of hazards.
· There are no weapons present at the facility as stated by Director Maria Lara.
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Hazardous items are stored where inaccessible to children.
· There are sufficient outdoor age-appropriate toys, equipment on the playground.
· A Staff member is present with current Pediatric CPR/First Aid which expires on 06/07/2026 which does not have EMSA stickers.

· Opening and closing staff member’s CPR/First Aid expires on 06/07/2026 which does not have EMSA stickers.
· Director completed Health and Safety Training in 1989 - college credits
· A review of staff personnel on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The application for an increase in capacity will be submitted for approval with a maximum capacity of 39 once the following corrections have been submitted.

Correction(s) needed:


1. Director is required to complete the orientation training for Application and record keeping.
2. EMSA stickers need to be obtained for the CPR & First Aid Training's OR staff is required to renew CPR & First Aid training from an approved vendor.
3. Clear the items located in restroom inside Room A – bathtub area and under the sink.
4. Provide step stool for children’s use in the restroom – for the adult sized toilet & the sink.

Once all corrections have been verified, the application for an increase in capacity will be submitted for approval with a maximum capacity of 39 Preschool children.
Director was advised that all corrections are due within 30 days, or the application may be withdrawn.

A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Exit interview conducted and report was reviewed with the licensee, Director Maria Lara.
