Licensing Program Analyst (LPA) Morris conducted interviews with the Site Director and staff regarding the allegation that a pest was found in a child’s diaper due to staff neglect. All individuals interviewed denied ever seeing or hearing of a pest—specifically a roach—being found in a child’s diaper at the facility. Further interviews revealed that neither the Director nor the staff were aware of such an incident, and it had not been reported or brought to their attention. Based on the information obtained through these interviews, the allegation could not be corroborated. There was insufficient evidence to determine whether a pest was present in a child’s diaper during a diaper change at the facility.

The facility provided LPA with 13 months of service receipts, dated from 05/28/2022 to 04/03/2025, confirming regular pest control visits. Site Director Yulisa Medina also reported that a professional cleaning company services the facility nightly to maintain cleanliness and sanitation. Additionally, LPA conducted a physical inspection of the facility and did not observe any evidence of a pest infestation. Based on the information obtained through interviews, documentation, and facility inspection, there is insufficient evidence to support the allegation.

Based on the information obtained through interviews and documentation provided, the allegation could not be verified. There is insufficient evidence to substantiate that staff failed to maintain the facility free of pests. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the allegations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

An exit interview was conducted and a copy of this report along with the appeal rights were provided to Site Supervisor Yulisa Medina. A notice of site visit was provided and must remain posted for 30 consecutive days.
