32 | Based on interviews conducted with multiple staff members, it was determined that the children were waiting in the hallway alone for at least two minutes. They were later observed by a staff member, who was returning from break, near the school-age classroom. When asked why they were in the hallway, the children stated they were waiting for their authorized representative.
Current staff stated, under the approval of a previous staff member who is no longer employed at the facility, children had been permitted to gather their personal belongings and wait in the hallway for their authorized representatives during pick-up without the supervision of staff being present.
The facility disclosed the incident to the Department within the required 24-hour time frame. Additionally, a written incident report detailing the absence of supervision was submitted to the Department within seven days.
Based on interviews conducted, the Department has determined that the preponderance of evidence standard has been met. Therefore, the above allegation is found to be SUBSTANTIATED. Violations of California Code of Regulations, Title22, are being cited on the attached; see LIC 9099D for cited deficiencies.
An exit interview was conducted with Site Director Jennifer House. A copy of this report, the Notice of Site Visit, and Appeal Rights were provided. The Notice of Site Visit must remain posted for 30 consecutive days.
A Civil Penalty has been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH.
Also, LPA Aman Lama informed the Director, Jennifer House to provide a copy of this licensing report dated February 25, 2026, that documents any Type A citation(s) to parents/guardians of all children currently enrolled, or newly enrolled by the next business day or the next day the child(ren) is(are) in care. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification and kept on file for 12 months from the date of this report.
LPA Aman Lama informed Director Jennifer House that this report dated February 25, 2026, document(s) (1) Type A citation(s) which shall be posted for 30 consecutive days as there is(are) immediate risk(s) to the health, safety, or personal rights of children in care.
