25 | Licensing Program Analyst (LPA) Claudia Caywood arrived at the facility to conduct a Case Management inspection in response to the receipt of an Unusual Incident Report (UIR) from the facility dated April 8, 2024. It documents an incident involving C1 and C2 who were left outside unattended during a transition from outside playtime to classroom.
At the time of the inspection, LPA toured the facility, took census, and met with Jazmine Hernandez, site director. Interviews with pertinent parties were conducted to obtain details regarding the reported incident. During the facility tour the area in which the incident occurred was observed.
Interviews revealed that C1 and C2 were found outside at the playground unattended and found by S1. S1 immediately brought the children in to their classroom and made S2 aware of the incident.
Facility records were reviewed, and S1 and S2 interviewed. Based on the information gathered, the following violation was found. Children were left unattended while transitioning from playground to classroom.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted, appeal rights discussed, and report was reviewed with the site director, Jazmin Hernandez .
