On 07/18/2024 at 11:30 am Licensing Program Analyst (LPA) Diana Brasel and Licensing Program Manager (LPM) Monica Cuddy arrived at the facility as a follow-up visit for the concluded substantiated Unlicensed Care delivered on 7/05/2024. Manzar Farooqui advised LPA Brasel on 07/15/2024 that the facility was no longer operating. Upon arrival today, LPA and LPM obtained information that children were present at the facility this week to included 07/17/2024. LPA and LPM rang door bell and ring camera, LPA and LPM observed through the window Manzar Farooqui in the facility, the lights were on, LPM saw through the window a female adult with two children. Manzar Farooqui would not answer the door after seeing LPA and LPM nor would he respond to the telephone call placed or the message left on the ring. LPA and LPM observed Manzar Farooqui walking away from the facility. LPM and LPA approached Manzar Farooqui, after a conversation Manzar Farooqui granted access to the facility. Upon initial conversation, Mr. Farooqui stated, he was closed, then changed his statement to say there were two children inside napping. Upon entry LPA and LPM observed a total of 14 children napping in one classroom with two adults.

Based on LPA and LPM's observations, interviews conducted, and documentation obtained there is a preponderance of evidence that Mr. Farooqui has continued to provide unlicensed childcare after 07/05/2024 when he was provided a copy of the substantiated unlicensed complaint report and a Notice of Operation in violation of law.

LPA and LPM attempted to conduct an exit interview with Mr. Farooqui. Mr. Farooqui stated, he did not want to read the report and refused to sign the reports. Civil Penalties were assessed, and a second Notice of Operation In Violation of Law was issued today.

No person, firm, partnership, association, or corporation shall operate, establish, manage, conduct, or maintain a child day care facility in this state without a current valid license therefor as provided in this act. This requirement was not met as evidenced by LPA and LPM’s observation of 14 children

Owner must immediately cease operation. Mr. Farooqui understands that civil penalties will continue to accrue until all operation ceases and that continuing to provide child care without a license may result in additional civil penalties, and/or criminal action being taken against Mr. Farooqui.
