On 7/282022 .at 9:35 am, Licensing Program Analyst (LPA) Patricia Berry conducted a subsequent complaint investigation. LPA was granted access into the facility and met with Jazmine Hernandez. LPA toured facility and took a census.

It was alleged, at times, a child was left in a soiled diaper and is not cleaned properly during diaper changes. During the investigation, LPA conducted interviews with all pertinent parties and obtained daily and sign in/out reports for the children from July 11, 2022 to July 21, 2022.

LPA reviewed the daily reports for the child. The daily reports state the times when staff changed the child’s diapers and if the child’s diaper was either dry, wet, or if the child had a bowel movement. In addition to the daily reports, staff document the same information for all children on an App called “Tadpoles”.

The “Tadpoles” reports for the children are sent directly to the guardian/parent’s cell phone. Staff stated children diapers are changed every two hours or as needed.


Based on interviews conducted and documentation obtained, due to conflicting information obtained, LPA was unable to determine if the child was left in a soiled diaper or if the child is not cleaned properly during diaper changes. This agency has investigated the complaint allegations. Although the allegations may have happened, or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.


Exit interview conducted with director, report, appeal rights and Notice of Site Visit issued.
