Upon entry into the facility, LPA observed S2 and S3 who were present and do not have current criminal record clearances. Interviews conducted disclosed S2 and S3 have resided in the facility since 12/28/2024 without current criminal record clearances while care was being provided to children. Records reviewed revealed both S2 and S3 listed as additional tenants residing in the home. This is an immediate risk to children in care and is an immediate civil penalty of $200. LPA reminded Licensee that all adults are required to obtain a current criminal record clearance prior to initial presence to the facility.
Based on observations made at the facility, information obtained during interviews, and records reviewed, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12, Chapter 3), are being cited on the attached LIC 9099-D.
LPA Tiffanie Diep informed the licensee, Miaohong Sun, that this report dated 01/23/2025 documents one Type A citation which shall be posted for 30 consecutive days as there was an immediate risk to the safety of children in care.
Also, LPA Tiffanie Diep informed Licensee to provide a copy of this licensing report dated 01/23/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgment of Receipt of Licensing Reports (LIC 9224), or other written statement, must be placed in the child’s file for verification.
An exit interview was conducted and report was reviewed with the licensee, Miaohong Sun. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. A copy of Appeal Rights (LIC 9058) in Chinese was provided to Licensee. |
