32 | Based on LPA’s observations and interviews which were conducted and reviews, the preponderance of evidence standard has been met, therefore the above allegations that the facility is not following reporting requirements, that a child was left unattended and that a staff handled children in a rough manner, are found to be SUBSTANTIATED. California Code of Regulations, Title 22, divisions, 101212(d)(1) Reporting Requirements, 101229(d)(1), Reporting Requirements 101229(f), Responsibility for Providing Care and Supervision 101229(a)(1) and Personal Rights 101223(a)(3) are being cited on the attached LIC9099D pages.
If a Civil Penalty has been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH.
LPAs Susan Brewer and Laura Mejorado informed Program Director Marissa Jaimes-Enriquez ,that this report LIC9099D dated 03/10/2026 documents 2 of Type A citation, which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPAs Susan Brewer and Laura Mejorado informed the Program Director Marissa Jaimes-Enriquez, to provide a copy of this licensing report dated 03/10/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A civil penalty was issued in the amount of $500.00.
A notice of site visit was given and must remain posted for 30 days.
