On 08/26/2025 at 2:10 PM Licensing Program Analysts (LPAs) Susan Brewer and Laura Mejorado, arrived at the facility to conduct an annual inspection. LPA was greeted by Licensee Guadalupe Gomez-Quintero, and granted entry to tour the facility inside and out. LPA reviewed records and observed and/or discussed the following: The requested the LPAs to inspect and additional unit, to operate the daycare. A fire clearance inspection was conducted for the additional playroom near dining room and kitchen and passed inspection on 08/11/2025. Present were the licensee and 3 minor residents included in the census on today’s date.

Days and hours of operation: Monday-Friday, 7:00 AM to 6:00 PM
OFF-LIMIT AREAS INCLUDE: Master bedroom/bathroom #1, Bedrooms #2, #3, Garage, the Backyard, additional play room with restroom #.

The inspection consisted of reviews of the following domain: Physical Plant, Care and Supervision, Records, Facility Administration, Staffing Ratio and Capacity, Personal Rights. The inspection found the facility to be in compliance in these domains, except as noted on the LIC809D.

· The Licensee is present in the home and has ensured that children in care are supervised.
· The facility is operating within the licensed capacity and appropriate ratios. LPA took a census of 8 children in care.
· When temporarily absent from the home, the Licensee shall arrange for a substitute adult to care for and supervise children.
· A working telephone is present.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

· A fully charged fire extinguisher (2A:10BC) was observed and tagged by the fire department and needle in the green. A smoke detector and carbon monoxide detector were present and tested by the licensee during this inspection on 08/26/2025.
· All hazardous items are NOT inaccessible, this includes detergents, cleaning compounds, medications and other items which could pose a danger to children. The LPAs observed cleaning supplies stored on top of the refrigerator such as Fabuloso, Lysol, Microban, Soft Scrub, Oven Cleaner, and other household detergents. In addition, the LPAs observed medications unlocked in an upper kitchen cabinet. The LPAs did not observe any children in the kitchen at the time of the inspection. The licensee removed the items in the presence of the LPAs and relocated the items to a secure area.
· Storage of poisons are NOT inaccessible to children and are NOT locked as mentioned above.
· This is single-story home, no stairs observed on this date.
· The fireplace is properly screened.
· No guns or weapons present as stated by the Licensee. Licensee Guadalupe Gomez-Quintero understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 regulations.
· Home is clean and orderly, with heating and ventilation for safety and comfort.
· Clean, Safe and age-appropriate toys and equipment are present for both indoor and outdoor activities.
· Outdoor play areas are fenced and/ or appropriate supervision is present.
· Verification of control of property on file by rental agreement.
· Property owner/landlord notification and consent on file by: Documentation on file.
· Pediatric CPR training on 03/16/2024, exp. 03/10/2026; 1st Aid exp. 04/02/2024 and training not renewed.
· Health & Safety Certificate - completed on 01/27/2020, no lead.
· Mandated reporter General: 06/04/2024 AB 1207 Child Care Expires: 06/04/2026.
· Fire clearance: 07/18/2019; last fire inspection 08/11/2025 to inspect the additional playroom near dining room and kitchen. The facility passed inspection, however the LPAs did not approve the activity area with the attached restroom on today’s date. The playroom and restroom is currently off limits and still being set up. LPAs observed power tools, hardware supplies, equipment not installed, batteries and craft supply cutter unlocked and accessible if children were present. The activity area will be re-inspected upon completion of the space and when hazardous materials are removed. The licensee stated they will notify the department once the area is completed and ready.

· Documentation of fire & earthquake drills to be conducted every six months: Last fire drill in May 2025, no day in the month documented.
· There are bodies of water 08/26/2025, in the outdoor play area where the licensee has a water activity table. No children were present at the time of observation. The Licensee Guadalupe Gomez-Quintero, stated the children were playing with the water table today. The licensee emptied the water table in the presence of the LPAs. The licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Children’s files are NOT complete on today’s date. The sleep log for an infant in care is missing name, and 15 minute checks, documentation of checks for labored breathing, temperature and sleep status.
· Staff’s files are complete on 08/26/2025
· A review of staff records on 08/26/2025 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Licensee Guadalupe Gomez-Quintero, was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPAs Susan Brewer and Laura Mejorado, discussed the safe sleep regulations with Licensee Guadalupe Gomez-Quintero and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. The licensee stated there are no infants enrolled at this time.

LPAs also informed Licensee Guadalupe Gomez-Quintero, of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
