1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 04/11/2024 at 11:00am, Licensing Program Analyst (LPA), Diana Brasel conducted an announced Pre-Licensing inspection for a Change of Ownership. Upon arrival, LPA met with Rosie Perez Director/Facility Representative. Applicant is requesting to be licensed for 66 pre-schoolers ages 2 - 5 years of age in classrooms #1, #2, #4, #5, and #6 and 6 toddlers ages 18 months - 36 months in classroom #3. Hours of operation will be Monday - Friday 6:30 am - 6:00 pm. Fire clearance was granted on 12/12/2023. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Rosie Perez that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Rosie Perez stated, it will stored in the main office. Per Rosie Perez medication that needs refrigeration is not accepted, if a child needs this type of medication the parent will be required to administer. A fully equipped first aid kit is located in the hallway. There is an operational carbon monoxide detector on site located in the hallway. All required licensing documents were observed posted in the lobby. Children will be signed in and out in the lobby. LPA continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured 2728.80, which is sufficient to accommodate the requested capacity of 72 Children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, cribs/napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via filtered bottle water, children bring their own water bottle filled. The children's water bottles are used for outdoor play. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property.

LPA observed a total of 6 sinks and 6 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 72 children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be in the isolation room as noted on facility sketch.

Facility will provide lunch and am/pm snacks. The kitchen area currently includes sink, refrigerator, dishwasher, stove, and microwave. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via a locked door.

The facility currently has a fully fenced playground area. Fencing wrought iron fencing is at least four feet high. The total square footage for all the outdoor activity space is 9048.40, which is sufficient to accommodate the requested capacity. Shade is provided via sun shade sail square canopies. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a small climbing structure (Step 2 double slide climber) on the toddler playground for children ages 12 mos - 3 years. There is not adequate cushioning in fall zones of small climbing structure, see corrections needed. Currently there are a few padded cushioning squares that do not cover the fall zone for a child. LPA observed all hazardous items on the playground to be inaccessible to children.

Applicant Rosie Perez was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

The following corrections are needed prior to the issuance of the license:
1. Playground 3 (toddler option) proof of added cushioning material has been added around the small
climbing structure.
2. Playground 3 (toddler option) proof that the Blue Choo Choo all exposed rust has been repaired.
3. Room 3 and room 4 proof that the diaper drawers have been anchored. Cubby in hallway outside of
room 3 has been anchored.
4. Room 5 proof that the small hole in wall has been repaired.
5. Playground 1 proof that the bicycle merry-go-round has been repaired removing all rust and the
replacement of one pedal.
6. Proof that the large hole on the PS additional grass area playground has been filled.
7. Proof that all wrought iron fencing surrounding the multiple playgrounds has had all rust areas
repaired.
8. The completed LIC 404 needs to be received from the designate bank.

Facility Representative understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.

Criminal Record Clearance - Child Care Centers:
Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.”
