On the date and time listed above, Licensing Program Analyst (LPA) Giselle Carbullido arrived at the facility for the above complaint received on 09/05/25. LPA was given access to the facility by Patrisia Gonzalez, Director. LPA discussed the purpose of the visit, took a census, and toured the facility. LPA met with the Director to further discuss the complaint allegations and deliver findings.
It was alleged the facility does not maintain fire clearance as licensed for room two (building two). During the investigation, LPA interviewed reviewed records, including the facility file, toured the facility, and interviewed pertinent parties.
A review of the facility file revealed the fire clearance and license notes the following: Room 2 is approved for 24 infants per fire clearance. During the tour of facility on 09/09/25, at 11:15AM, LPA obtained a total census of 26 infant children.
LPA reviewed a sampling of the following facility records from 08/04/25 to 09/09/25: face to name transition sheets; daily health and head count sheets and a monthly classroom attendance log. Records revealed room two has had more than 24 infants present.

Interviews with relevant parties stated room two often operates between 26 to 28 infant children. Additionally, interviews disclosed the room two feels “cramped” when there are more than 24 infant children.
Based on interviews conducted, the Department has determined the preponderance of evidence standard has been met; therefore, the above allegations are found to be SUBSTANTIATED, per the California Code of Regulations, Title 22, Division 12. See LIC9099D for deficiency cited; a civil penalty has been issued in the amount of $500.00.
If a Civil Penalty has been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH.
LPA Carbullido informed facility representative, Patrisia Gonzalez that this report dated 09/15/25 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Carbullido informed the facility representative, Patrisia Gonzalez to provide a copy of this licensing report dated 09/15/25 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

An exit interview was conducted, and appeal rights discussed. LPA provided facility representative, Patrisia Gonzalez with a copy of this report, notice of site visit and appeal rights.

Based on LPA’s record review and interviews conducted, the facility did not comply with the section cited above in maintaining the fire clearance approved for 24 infants. This is an immediate risk to the health and safety of children in care. A civil penalty has been issued in the amount of $500.00.
