On the date and time listed above, Licensing Program Analyst (LPA) Giselle Carbullido conducted a subsequent complaint investigation to deliver final findings. An initial visit was conducted on 04/22/26 at which time LPA discussed the purpose of visit; toured the facility and conducted interviews. LPA met with facility representatives, Patrisia Gonzalez- Director and Julia Montana- Assistant Director; toured the facility and obtained census.
During the investigation, LPA reviewed records and interviewed pertinent parties, including facility staff and subject child.
It was alleged that facility staff did not ensure supervision was provided to children at all times. It was reported two school-age children were left alone outside in the parking lot. Staff and child interviews corroborated an absence of supervision occurred when children were left unsupervised by a vehicle in facility's front parking lot.
Based on interviews conducted, the Department has determined the preponderance of evidence standard has been met; therefore, the above allegations are found to be SUBSTANTIATED, per the California Code

LPA Carbullido informed Patrisia Gonzalez and Julia Montana facility representatives, that this report dated 05/06/26 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Carbullido informed the facility representative, Julia Montana to provide a copy of this licensing report dated 05/06/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
An exit interview was conducted, and appeal rights discussed. LPA provided facility representative, Julia Montana with a copy of this report, notice of site visit and appeal rights. LPA observed the Notice of Site Visit was posted by staff. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC FOR THREE YEARS
