32 | On October 31, 2025, a child was dropped off at the facility wearing a Halloween costume however, upon being picked up at the end of the day, the child was no longer wearing the costume. Initially, staff were unaware of how or when the child removed their costume and could not locate it. A message was sent, via Brightwheel application, to all parents/legal guardians of children that had picked up their children on that day. The costume was not found on that day. It was later determined that the facility Director had removed the costume from and placed it in another child’s backpack/cubby. The following week, on a Monday, the costume was returned by another parent/legal guardian and reimbursement was issued.
Based on the information obtained, and although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is unsubstantiated.
A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
