On this date and time, Licensing Program Analyst (LPA's ), Diana Brasel and Griselda Castellon conducted an announced follow-up Pre-Licensing inspection to add a toddler option component to the existing licensed facility. On today's visit LPA's measured the outside playgrounds for the infant and toddler option programs. The initial visit was conducted on 02/24/26. The facility currently holds a preschool and infant license, facility numbers are 334846181 and 334846182. Upon completion of the toddler option component the facility will be a single license conversion. Upon arrival, LPA met with licensee Aruni Geekiyanage and Hasith Hirikandeniye licensee representative. Licensee is requesting to be licensed for (62) preschoolers (2 - 6) years of age in rooms 3, 4, and 5. (30) toddler option children ages 18 - 36 months in room 2. (16) infants ages 6 weeks - 24 months in room 1. Hours of operation will be Monday- Friday 6:30am - 6:00pm.

Total indoor activity space was measured on 02/24/26 for the infant component is 744.14, which is sufficient to accommodate the requested capacity of (16) children. Total indoor activity space measured for the toddler option component is 1285.04, which is sufficient to accommodate the requested capacity of (30) children. Total indoor activity space measured for the preschool component is 2428.58, which is sufficient to accommodate the requested capacity of (62) children.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The facility currently has a fully fenced playground area. Fencing is a block wall and chain link fencing, which is at least four feet high. The total outdoor square footage for the infant outdoor activity space is 1352.04, which is sufficient to accommodate the requested capacity. The total square footage for the toddler option outdoor activity space is 1197.91, which is insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure). The total outdoor square footage for the preschool outdoor activity space is 4761.43, which is sufficient to accommodate the requested capacity. Shade is provided via a large built in canopy over the outdoor infant playground. The licensee stated, shade will be added to the preschool and toddler option playgrounds. (see correction needed) There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the preschool playground which has been cleared previously for preschool children and is properly anchored. There is adequate cushioning in fall zones of climber provided by artificial turf with rubber underneath and large rubber mats located a the bottom of the slides. The climbing structure located on the toddler option playground is for ages 1-3. Drinking water is available via children's brought water bottles. LPA observed all hazardous items on the playground to be inaccessible to children. Licensee was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

The following corrections were needed from the 02/24/26 visit. LPA's verified during today's visit that all below corrections have been completed.
Infant room:
1. Proof that the wall repairs in the activity room leading to the outdoor playgrounds have been completed.
2. The areas of the wall in the activity room leading to the outdoor playgrounds that have broken dry wall shall be repaired.
3. The bottom section of the door located in the activity room leading to the outdoor playground shall be repaired to prevent access to the exposed compressed wood.
4. Proof that an additional children's sink has been added.
Toddler Option room:
1. Proof that the started repair/patch work to the small area on the wall located on the left wall from front area of room has been completed.
Playgrounds:
1. Proof that the rust on the wrought iron fence around the infant playground has been repaired or replaced.

As of today's visit the following corrections are needed prior to the issuance of the single license:
1. Proof that the additional shade has been added to the preschool playground.
2. Proof that the fence surrounding the toddler option playground shall be increased in height to measure 4 feet all around. Currently the height measures 3 ' 2".
3. Proof of shade being added to the toddler option playground.
4. Proof that the fencing surrounding the infant playground shall measure 4 feet all around. Currently only one panel measures 4 feet.
