Regarding the allegation that staff did not ensure the facility was free of rodents; LPA toured the area addressed in this allegation and did not observe any rodents or evidence of rodent droppings.

Executive Director McAdoo-Morton produced invoice from their contracted Pest Control indicting that bait traps were placed throughout the facility and no observation of mice activities were observed on the date of pest control visit (5/22/2024). Director also states that issues of mold, odor or rodents were not brought to his attention prior to LPA complaint visit.

Based on the information obtained there is not enough evidence that staff did not ensure the facility was free of mold, staff did not ensure the facility was free of odor and staff did not ensure the facility was free of rodents. Therefore, the allegations are deemed UNSUBSTANTIATED at this time. This report was signed by LPA Prieto and Executive Director McAdoo-Morton and a copy was left with the facility.
