During the inspection at 1:57PM, LPA found one infant asleep in a swing that was placed in a pack and play. The child’s body was covered with one blanket and the child’s face was covered with another blanket. There were four blankets hanging on the pack and play with one blanket placed on each side. There were approximately 15 loose toys in the pack and play with the sleeping infant. LPA also found a pacifier attached to the sleeping infant. There was no Infant Safe Sleep Plan (LIC 9227) in the child’s file. Both the swing and pack and play were not being used per manufacturer instructions.
