25 | On October 8, 2025, at 11:49 AM, Licensing Program Analyst (LPA) Courtnee Peebles conducted an unannounced Case Management visit to follow up on an Unusual Incident Report (UIR) submitted by the facility on October 2, 2025. The incident reportedly occurred on both September 30, 2025, and October 2, 2025. Upon arrival, LPA Peebles met with the Facility Director, Paula Shigo, to discuss the details of the reported incident.
The Director reported the following:
On October 2, 2025, at approximately 7:30 AM, while in care, Child #1 (C1) asked Staff #1 (S1) if Staff #2 (S2) would be coming to work. When S1 responded that S2 had not yet arrived, C1 stated, “Good, because S2 pushes me in the face.” It was later disclosed that on September 30, 2025, C1 had also told S2 that Staff #3 (S3) “yells and slaps me in the face.”
LPA conducted interviews with the Director and Parent #1 (P1), both of whom confirmed that C1 has a history of making false statements in an attempt to elicit reactions from others. There is no evidence to support the allegations made by C1, and the incidents described were determined not to have occurred.
Based on the information obtained during the visit, LPA concluded that the facility responded appropriately and took the necessary actions to ensure the safety and well-being of the children in care. There are no identified violations of Title 22 Regulations in relation to the reported incident.
An exit interview was conducted with the Director, and a copy of this report was provided.
A Notice of Site Visit was issued and must remain posted at the facility for 30 days.
