On November 1, 2023 at 12:45 pm, Licensing Program Analyst (LPA) Jessica Rubio arrived at the facility to conduct an annual inspection as part of a compliance review. LPA met with Licensee Janelly Montano, toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed:
· Normal days and hours of operation are: Monday through Friday 8:00 am to 5:00 pm. On this date, the facility was closed and has been closed since 8/30/2023 and is expected to re-open on 11/13/2023.

· Off-limit areas include: The entire home, except the daycare area (mother-in-law suite).

· The facility is licensed to have no more than 14 children as a large family child care home and is operating within the licensed capacity and appropriate ratios. LPA observed only, one child the licensee’s own child in care. Licensee stated only one other child is enrolled at this time.

· A working telephone is present, and the current phone number is on file

· A fully charged fire extinguisher (2A:10BC) was observed. A smoke detector and carbon monoxide detector were present and tested by the Licensee during this inspection.

· Weapons are not present on this date as stated by licensee. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations

· Stairs are barricade when operating. LPA observed the barricade.

· Clean, safe, and age-appropriate toys are provided

· Facility Sketch, Emergency Disaster Plan and Notification of Parent’s Rights poster are not posted as facility has not been operating, however, LPA advised to place posters back up prior to re-opening.

· Documentation of fire and disaster drills are not on file – Licensee stated the last drill was conducted in June 2023. LPA advised licensee of the requirement to document.

· No bodies of water are present at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.

· Verification of control of property is on file

· LPA reviewed one child’s file. The child’s record was incomplete. Required immunizations were not documented and child was missing the LIC 627 Consent for Emergency Medical Treatment. Citations will be issued. Child was also missing signed personal rights form LIC 995A and LIC 282. Technical violations will be issued.

· Licensee Janelly Montano’s Mandated Reporter Training expires on 3/6/2024

· Licensee Janelly Montano’s Pediatric CPR and First Aid Card expires on 3/16/2024

· Health & Safety Certificate - completed on 4/10/2022


· Resident and/or staff records were reviewed and all adults who require caregiver background checks have received all required clearances and/or exemptions.

Licensee reminded that all adults 18 and over, living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The Licensee was informed of their reporting requirements and was provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov

During the exit interview, the Licensee Janelly Montano, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. An exit interview was conducted, and this report was reviewed with the Licensee Janelly Montano. Appeal rights were discussed and provided during the exit interview. A notice of site visit was given and must remain posted for 30 days.
