25 | On May 14, 2024, at 11:56 AM, Licensing Program Analysts (LPA's ) Anastasia Flores and Courtnee Peebles, met with Licensee, Nam-Desilva, and Director, Leslie Marquez, to discuss an unusual incident report (UIR) that was reported to Community Care Licensing office on May 2, 2024. Our agency was made aware of an incident that occurred on 4/26/24 where a child #1 (C1) swallowed some sand from the playground and licensee was informed that C1 was in the hospital due to lead in the sand at the facility. The facility provided records from department of health and lead poison prevention team dated 05/06/24, stating the preliminary sand collected did not detect any lead from the sand.
As of this date, 5/14/24, the facility has no deficiencies in relation to this UIR.
An exit interview was conducted, and a copy of this report was provided to Director, Leslie Marquez..
A notice of site visit was also provided and must remain posted for 30 days.
