On March 7, 2025, at 12:35 PM, Licensing Program Analyst (LPA) Brian Morris arrived at the facility for the purpose of delivering findings on compliant investigation initiated on 12/20/24. LPA took census of children in care and staff onsite. LPA initially met with Licensee Erica Udoffia and Enoh Udoffia on 1/02/2025, LPA also took a tour of the facility and observed supervision of children in care transportation van, transportation van booster seats, transportation van car seats, reviewed files, and conducted in person interviews with licensee’s, minor children, licensee’s minor children and observed children in care.

In response to the allegation, licensee does not ensure children are safely secured in a restraint system while being transported, LPA was unable to corroborate allegation. During investigation, LPA inspected the vehicle used for transport and confirmed the vehicle was equipped with the necessary safety features, such as seat belts and other systems to ensure passengers were securely restrained.

LPA also observed that the vehicle had booster seats and car seats inside, including additional booster seats and car seats inside the garage area of the facility to use if necessary. In addition, LPA conducted interviews with pertinent parties and was unable to corroborate allegation.

In response to the allegation, child sustained an unexplained injury while in care, LPA conducted interviews with staff and children and was unable to corroborate allegation. During interviews conducted, the licensee reported that all diapering in the facility was handled by them to prevent any potential accusations. LPA observed that the licensee had long fingernails, but denied injuring the child and was not aware the child had sustained any injuries. Due to reporting party being anonymous, LPA was unable to obtain any evidence or gather pertinent information regarding the allegation.

In response to the allegations that the licensee allows children to be left without supervision, the claim will be closed as unsubstantiated. During interviews, the licensee denied the allegation, and interviews with the children in care, including the licensee's own children, confirmed that an adult was always present to provide supervision, and they were never left alone. Due to a lack of evidence, the allegation could not be substantiated.

Based on investigation findings, the allegation that the licensee does not ensure children are safely secured in a restraint system while being transported, child sustained an unexplained injury while in care, licensee allows children to be left alone without supervision while in care, may have occurred, however is not supported, or proven by evidence. Therefore, the allegations are Unsubstantiated.

An exit interview was conducted and a copy of this report, appeals rights and notice of site visit were provided to licensee.

The Licensee, Erica Udoffia, and licensee Enoh Udoffia were reminded that the notice of site visit must remain posted for 30 consecutive days.
