On August 26, 2026, at 10:00 AM, Licensing Program Analyst (LPA) Courtnee Peebles arrived at the facility to conduct a case management visit to follow up on an Unusual Incident Report (UIR) that occurred on August 21, 2026. LPA toured the facility inside and outside with Principal Misty Kelley, interviewed one staff member, and observed the area where the incident took place.
The incident occurred on August 21, 2026, at approximately 3:13 PM in classroom D104, involving a child who sustained an injury requiring medical attention. According to the principal, it was cleanup time in preparation for dismissal when Child 1 (C1) began running, tripped, and hit their head on the writing center desk. C1 immediately stood up, held their head, and began crying. Staff 1 (S1) responded right away and observed a large red bump on C1’s head, approximately the size of a golf ball. A second bump began forming, prompting S1 to call for additional staff to assist with a risk assessment and provide ice.
At approximately 3:18 PM, C1 began repeatedly yawning, and staff attempted to keep the child awake. Additional staff contacted 911 for emergency services. At 3:24 PM, C1 lost consciousness, and Staff 2 (S2) monitored the child’s breathing while Staff 3 (S3) provided guidance over the phone while en route to the center. Paramedics arrived at approximately 3:35 PM, administered emergency medical care, and C1 regained consciousness. The child was transported to the hospital and treated for their injuries. C1 has not yet returned to the facility pending medical clearance.Facility staff have taken preventative measures by rearranging classroom furniture to help prevent similar incidents in the future.
An exit interview was conducted,and a copy of this report was provided to the Principal, Misty Kelley. A Notice of site visit was provided and must remain posted for 30 days.
