Some staff reported that parent 1(P1) had previously become frustrated by the number of incident reports provided, and staff therefore gave verbal updates instead of written documentation. Later P1 requested written documentation for all incidents involving C1. While the investigation found inconsistencies in communication, there was no evidence that staff intentionally withheld required documentation or that this resulted in a health or safety concern.
Additional parent interviews confirmed that the facility communicates through an application and provides written incident reports through that platform. Some parents noted that minor incidents occasionally went unreported; however, no injuries or significant safety concerns were disclosed.
Based on observations, records reviewed, and interviews conducted, there is insufficient evidence to support the allegations that staff failed to follow napping procedures or did not provide required incident reports. Therefore, both allegations are unsubstantiated. Although the allegation may have occurred or may be valid, there is not a preponderance of evidence to prove the violations did or did not occur.
An exit interview was conducted, and Appeal Rights were provided. A Notice of Site Visit was issued and must remain posted for 30 consecutive days. The licensee was reminded that the Notice of Site Visit must be posted in a prominent location, visible to parents and caregivers. Failure to post the notice will result in a civil penalty of $100. |
