On April 29, 2024 at 9:45 AM, Licensing Program Analysts(LPAs), Ana Noble and Diana Brasel conducted an announced Pre-Licensing inspection for a New License. Upon arrival, LPAs met with Applicant Tyra Hawthorne. Applicant is requesting to be licensed for 48 preschoolers, ages 3-5 in Classrooms 1, 2 and 4. Hours of operation will be Monday-Friday 6 am-6 pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Hawthorne that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, stated that they will be in the Director's office in the storage closet. A fully equipped first aid kit is located in upper cabinets in Classroom 1. There is an operational carbon monoxide detector on site located in each classroom 1, 2 and 4. All required licensing documents were not observed posted in the lobby near the director's office. Children will be signed in and out at the front hall way between the reception area and the director's office.

LPA continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured 1746.43 Square Footage, which is sufficient to accommodate the requested overall capacity of 48 Children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via Britta Pitchers with children's own Re-useable cups. LPA did not observed all hazardous items to be inaccessible to children. There is a swimming pool on the premises which is not visible from the building. It has a wrought iron fence, which is at least 5 feet in height, and is self latching and self closing at this time.

There are no weapons on the property. Fire clearance was granted on 4/16/2024.

LPA observed a total of 6 sinks and 4 toilets available for children’s use. These are sufficient to accommodate the requested overall capacity of 48 children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the Director's office and bathroom.



Facility will provide breakfast, snacks and lunch. The kitchen area currently includes refrigerator, microwave and dishwashers. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via locked door requiring key to open.

The facility currently has a fully fenced playground area for each component. Fencing is a stucco block wall which is at least four feet high. The total square footage for all the outdoor activity space for the preschool is 5689.47, which is sufficient to accommodate the requested capacity of 48, there is no shade structure at this time. There are sufficient outdoor age-appropriate toys and play equipment available on the playground(s). There is a climbing structure on the Preschool playgrounds 1/2 and 3/4 for children ages 3-5, The Step2 Compamy LLC. which is properly anchored. There is adequate cushioning in fall zones of climber provided by rubber mulch. Drinking water is available via Britta Pitcher and children's own re-useable cups/dispensers. LPA observed hazardous items on the playground(s) which will be fixed and corrected as agreed to by applicant. Applicant Tyra Hawthorne was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).



Applicant, Tyra Hawthorne was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers,

A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with applicant the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

LPA also informed applicant of the importance of checking for recalled items and devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant, Tyra Hawthorne was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The following corrections are needed prior to the issuance of the license:
1. Parent Board with all the required forms, submit a pictures of parent board with all required postings and the location.
2. In Room 4, fix the broken door handle to one of the children's restroom, submit pictures of all corrections.
3. In Room 1, Fix the A/C vent on the ceiling which is falling and water damage by window, submit pictures of corrections.
4. In all classrooms make supply closets inaccessible, submit pictures of corrections.
4. In all 3 playgrounds, add shade structure and submit pictures of correction.
5. In playground 1/2, fix/repair the exposed metal on all the playground equipment and cracked slide, submit pictures of corrections.
6. In the bike route - playground fix the uneven concrete (tripping hazards), submit pictures of corrections.
