25 | On July 8, 2026, Licensing Program Analyst (LPA), Ahliah Sharp conducted an unannounced required annual visit. LPA was greeted by DSP Sonny Lim, who escorted LPA into the facility. Shortly after arrival, Administrator (ADMIN), Jeffrey Castillo, arrived and was informed of the purpose of the visit. At the time of the visit there were five (5) staff and three (3) residents present, the remaining two (2) were at day program.
The facility is a one-story home with four (4) client bedrooms, two (2) bathrooms, attached garage, and an outdoor space. The facility does not have a pool or any firearms. The facility is licenses as a residential care facility for adults ages 18 to 59. They are approved for six (6) non ambulatory clients.
Physical Plant: Physical plant, floors, windows, and doors were observed to be clean with all fixtures and furniture present and in good repair. The facility's outdoor area was observed to be free of hazards, and debris. Laundry equipment was observed to be in good working condition. LPA observed hazardous and potentially dangerous items, such as centrally stored medication, sharps, and cleaning supplies to be secured, locked and inaccessible to residents. Carbon monoxide, smoke detectors and fire extinguishers were charged and operational. The facility conducts monthly fire and earthquake drills, the last being conducted 6/14/2026, and 6/12/2026 respectively. Hot water temperature was recorded at 111F in the kitchen. The facility has secured cleaning supplies to conduct regular cleaning of the facility, and personal hygiene supplies for each resident.
Food Service: LPA observed facility kitchen has the ability to prepare food in a clean environment and possesses equipment in good working condition. LPA observed the facility met and exceeded the required two (2) day perishable food and seven (7) day non-perishable items.
