On 7/16/2026 Licensing Program Analyst (LPA) Edith Conchas conducted an unannounced visit to investigate on complaint allegations stated above. LPA met and discussed the reason for the visit with House manager, Jennifer Ulloa.

During LPAs’ visit today LPA conducted interviews, reviewed documents and a tour of the facility.

Staff is restricting residents' access to showers.
Interviews with two staff reveal that although there is a shower schedule, there are no restrictions on when the clients can or cannot shower. Interview with one staff revealed the shower schedule is in place to encourage the clients to keep up with their hygiene. Another staff interview reveals the shower schedule is not a rule. Based on interviews and observations, this allegation is UNSUBSTANTIATED.

Staff is not ensuring residents have adequate toilet paper.
Interview with three staff revealed that there is no limit to the toilet paper use and that toilet paper is replaced when it runs low. Two staff interviews revealed toilet paper is replenished before it runs out. One staff interview revealed the facility has extra stock. One client interview reveals there is one roll of toilet paper in the bathroom. LPA did observed a roll of toilet paper in the restroom and an extra stock available in the garage. Based on interviews and observations, this allegation is UNSUBSTANTIATED.

Staff do not provide a comfortable environment for residents.

LPA upon arrival observed the front door with screen open and back door with screen also open and observed 2 ceiling fans in the den area on. Interviews with three staff revealed that the air conditioning is turned on when the temperature gets warm. One staff interview revealed that even if the temperature is at 74 degrees Fahrenheit and the client reports feeling hot, staff will turn it on. Another staff interview reveals when the house temperature reaches 75 degrees Fahrenheit the AC is automatically set to turn off, if it get warm it turns back on. At approximately 10:00 am, LPA felt the AC turn on. Based on interviews and observations, this allegation is UNSUBSTANTIATED.

Based on interviews and observations, this allegation stated above are UNSUBSTANTIATED. Although the allegation may have happened or may be valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

All above allegations are UNSUBSTANTIATED. No deficiencies were cited at this time per Title 22, Division 6 of the California Code of Regulations.

An exit interview was conducted, the findings were reviewed, and a copy of this report was provided to House manager Jennifer Ulloa.
