Licensing Program Analyst (LPA) Stephanie Williams made an unannounced visit to the facility in order to conduct a complaint investigation into the above allegations. LPA met with Administrator, Marlya Dunham, and explained the purpose of the visit. The investigation consisted of interviews with residents and staff members and records review.

In regards to allegation #1, LPA interviewed Staff #1 (S1), Staff #2 (S2), and Staff #3 (S3) who stated that the facility offers various modes of transportation to appointments and activities for their residents. LPA also interviewed Resident #1 (R1) who stated that the facility has been offering transportation services to appointments and activities.

In regards to allegation #2, LPA interviewed R1 who denied that staff are intimidating them while in care. LPA interviewed Resident #2 (R2) who stated that they do not feel intimidated by staff members. LPA interviewed S1, S2, and, S3 who all denied that staff members are intimidating residents in care.

Based on evidence obtained during today’s visit, LPA has determined that the above allegations are UNSUBSTANTIATED; meaning that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

An exit interview was conducted where this report (LIC 9099 & LIC 9099C) was discussed and a copy was provided to the Administrator.
