Licensing Program Analyst (LPA), Stephanie Torres, conducted an unannounced visit to the facility after having been notified on February 15, 2022, of the death of Licensee, Armando Laygo. The LPA met with staff, Teresita Diaz, and spoke with Operations Manager, Cielita Ravelo, over the telephone. Ravelo was informed of the purpose of the visit.

Ravelo, who is representing the Corporation at this time, reported the facility operations will continue. She reported an updated LIC 309 is currently being updated and will be submitted to the Department. On this visit the LPA toured the facility and verified sufficient food supply. The LPA was unable to access two (2) storage sheds in the backyard. Ravelo was notified of the importance of staff having access to all areas of the facility.

This report was reviewed with Ravelo and a copy was provided.
