25 | On 08/25/2026, Licensing Program Analyst (LPA) Soleil Marx met with Facility Representative, Brittney Rand, for the purpose of conducting an unannounced annual inspection and case management inspection regarding lead testing. LPA observed a census of 18 toddler age children being supervised by five staff members in four separate toddler option classrooms, 10, 11,12,13. No preschool age children were present within the preschool classrooms, 2, 3, 4, 7, 8. Facility Representative was reminded that the facility shall not exceed the conditions, limitations, or capacity specified on the license. The facility’s hours of operation are Monday through Friday from 8:00 AM to 3:00 PM, with early release on Thursdays.
The purpose of today’s inspection was to develop a Plan of Correction following reports of lead exceedance in nine water outlets at the facility. On 04/01/2026, the facility conducted lead testing of water samples from various water outlets. The lead testing identified nine water outlets with lead levels exceeding 5.5 parts per billion (ppb). Water outlet B, located in classroom 2, had a lead level of 9.1 ppb. Water outlet E, located in classroom 4, had a lead level of 8 ppb. Water outlet H, located in classroom 7, had a lead level of 7.2 ppb. Water outlet M, located outside by the preschool restrooms, had a lead level of 7.3 ppb. Water outlet T, located in classroom 10, had a lead level of 7.3 ppb. Water outlet W, located in classroom 11, had a lead level of 27 ppb. Water outlet Y, located in classroom 12, had a lead level of 14 ppb. Water outlet Z, located in classroom 13, had a lead level of 5.6 ppb. Water outlet AA, located in classroom 13, had a lead level of 27 ppb. Water outlets B, E, H, M, and Z are temporarily inaccessible to children and have been covered with plastic while the facility completes flushing procedures. The facility will retest water outlets prior to returning them to use. Water outlets T, W, Y, and AA have been permanently capped off. LPA observed that uncontaminated drinking water was readily available to children both indoors and outdoors via alternate water sources. |
