On 05/15/2024, Licensing Program Analyst Katy Velazquez (LPA) conducted an unannounced field visit to deliver the findings for the above allegations. LPA arrived at the facility and was met by Director Rhonda Hansen Hensly (D1). LPA disclosed the purpose of the inspection and was granted entrance into the facilty. The facility was observed to have a census of 27 preschool aged children in-care.

Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, interviews, reviewed and collected documentation. LPA reviewed the facility’s file in the Regional Office and collected documentation pertaining to the allegations. It was alleged that a child was left unsupervised in the parking lot. Interviews with staff did not reveal corroboration for the allegation. Additionally, the parking lot has motion sensors with camera supervision. It was alleged that there is mold on the carpeted walls of the classrooms in the facilty. LPA toured the preschool and did not observe mold in any areas of the facility.

 It was alleged that the play yard is not fully enclosed with an opened fence. LPA observed a fully enclosed play yard with fences that open and close. Fences are not locked for fire safety. Interviews with staff, and LPA observations of children at recess, reveal that children are taught to not open the fences and to stay with their group. Additionally, recess is scheduled for small groups to ensure supervision on the play yard.

Based on interviews, observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegations are UNSUBSTANTIATED.

In the areas that were evaluated on 05/15/2024, no deficiencies were cited during today's inspection. An exit interview was conducted with Director Hansen Hensly and Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
