25 | Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct an annual inspection. LPA Moleski met with facility administrator Merlita James and explained the purpose of the visit.
LPA Moleski reviewed six resident files (R1-R6) and two staff files (S1-S2). S2's file did not contain a health screening. S2's training records for 2022 show S2 completed 12 hours of annual training, and for 2023, S2 completed four hours of annual training. S2 had no training records for 2024. S2's first aid/CPR certification, completed in December 2019, expired in December 2021.
R1 is diagnosed with dementia, according to R1's most recent LIC 602, which is dated 3/20/23. R3 is also diagnosed with dementia, according to R3's most recent LIC 602, which is dated 5/11/23.
LPA Moleski toured the facility with James and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas. Furniture and furnishings were sufficient to meet the needs of residents. The facility temperature was 75 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 108 degrees Fahrenheit, which is within the required range of 105 and 120 degrees. While touring the facility, LPA Moleski and James observed various automotive fluids and cleaning solutions left unlocked in the garage, which is unlocked and accessible from the interior of the facility. Then, while inspecting a bathroom, LPA Moleski and James observed that a lock on a cabinet was unlocked, allowing access to cleaning solutions kept under a sink. LPA Moleski observed first aid supplies, a fully-charged and up-to-date fire extinguisher, and carbon monoxide/smoke detectors. LPA Moleski observed a minimum 2-day supply of perishable food and a minimum 7-day supply of nonperishable food.
LPA Moleski interviewed one staff member (S1) and two residents (R4-R5).
