LPAs Melissa Lusby and Jacob Williams arrived on Thursday August 26, 2021 to conclude a complaint investigation regarding the above allegations. Prior to the visit, LPAs completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; LPAs ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N-95 mask and surgical mask. LPA was screened by the front desk prior. LPAs were screened upon entering the facility. Throughout the course of the investigation, LPAs reviewed the following documents: R1's physicians report, nursing notes, care plan, billing statements, and physicians orders. LPAs additionally interviewed the Administrator, Health Services Director, med techs, and direct care staff. Although staff were responsible for washing R1's personal laundry, the facility only has common area washers and dryers.

LPAs Melissa Lusby and Jacob Williams arrived on Thursday August 26, 2021 to conclude a complaint investigation regarding the above allegations. LPAs were screened upon entering the facility. Throughout the course of the investigation, LPAs reviewed the following documents: R1's physicians report, nursing notes, care plan, billing statements, and physicians orders. LPAs additionally interviewed the Administrator, Health Services Director, med techs, and direct care staff. LPAs learned that at the time of the complaint, the facility was not responsible for R1's medication or incontinence

management. Interviews revealed that R1 was able to toilet herself. Additionally, interviews revealed that if R1's bathroom was found to be unsanitary, staff would address the issue.

Based on LPAs interviews and review of documentation, the preponderance of evidence standards was not met, therefore, the above allegations are found to be UNFOUNDED. An unfounded allegation means that the allegation was false, could not have happened and/or is without a reasonable basis.

Therefore, LPAs were unable to determine if staff lost an article of clothing or another resident mistakenly took it. Through interviews, LPAs were unable to determine if R1's walker/basket belonged to another resident.

Therefore, the Department finds the allegations to be UNSUBSTANTIATED. A finding that the complaint allegations are UNSUBSTANTIATED means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.
