interactions between the facility staff and the residents in care. Based on interviews and direct observations, there is insufficient evidence to support the allegation.

It was alleged that staff are not ensuring residents’ incontinence needs are being met. The investigation involved observations and interviews with staff, residents and three family members of the residents in the home. During the inspections conducted on 07/02/2025 and again on today, 08/06/2025, LPA Lee observed no incontinence-related odors within the facility. Interviews were conducted with three out of three residents, but their responses did not clearly confirm whether staff are meeting their incontinent care needs, as each resident stated they manage their own briefs. Additionally, two facility staff were interviewed and denied the allegations, stating that residents’ briefs are checked every two hours and that residents are encouraged to use the toilet, and incontinence care is provided daily. In an interview with three family members of the residents, they stated that they had no concerns regarding the allegation and had not witnessed resident incontinence needs not being met. Based on observations and statements obtained during the investigation process, LPA was unable to corroborate the allegation.

The investigation revealed the preponderance of evidence standards have not been met; therefore, the above allegations are found to be UNSUBSTANTIATED. A finding that the complaint allegations are UNSUBSTANTIATED means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violation(s)occurred.

An exit interview was conducted, and a copy of this report was provided to the facility.
