13 | Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to complete the investigation and deliver findings to a complaint the Department received on 09/20/2021. LPA met with Doreen, caregiver, who contacted the Administrator/House Manager, Maria Williams, who arrived at 2:00 pm to the facility. Caregiver on site confirmed there are (6) residents currently, and there are no residents on hospice. Prior to initiating today's inspection, LPA completed required COVID-19 testing protocols and completed a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms. Additionally, LPA was screened per Covid-19 precautionary measures upon entering the community. LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: surgical mask. LPA observed (2) residents to be napping on the couch in the front room and (4) residents to be napping in their rooms.
During the course of the investigation, the Department interviewed the Administrator and House Manager and reviewed documentation, including, but not limited to, visitor signs posted at the front entrance, Personal Rights (LIC613-C), House Rules, Department Provider Information Notices (PIN), and visitor logs.
The results of the investigation are as follows: The complaint alleges that, on/around September 20, 2021, the facility posted signs stating the visiting hours are from 10:00-11:00 AM and from 2:00-4:00 PM only, there are no visits allowed after 5 pm, and there is only 1 person at a time for 20 minutes maximum.
