On September 8, 2022 at 9:47AM Licensing Program Analyst (LPA) Chris Hopkins arrived at this facility unannounced to conduct a complaint investigation regarding the above allegations. LPA met with Regional Director of Operations Scott Bissey and explained the purpose of the visit. Administrator Mike Talani is out today.

Regarding the allegation Residents not getting their medications on time during the night, the Department found the following; based on interview and record review, it was determined that residents are getting their nightly medications on time. LPA interviewed 4 residents and all stated that they receive their medication on time consistently. LPA also reviewed the MAR for those 4 residents and the MAR does reflect the residents receiving their medications.

Regarding the allegation Facility not doing anything about rat and mice problem, the Department found the following; based on interview and record review, it was determined that the rodent issue is outside at the garbage area away from the facility. Administrator has gotten a contract with Orkin to address this problem. Also Orkin has placed numerous bait traps around the garbage area. The facility itself does not have a rodent problem.

Regarding the allegation of Residents not supervised and leaving the building unnoticed, the Department found the following; based on interview, record review, and observation, it was determined that residents do not leave unsupervised. The facility is enclosed and residents have no way of exiting without supervision. If residents go out of the side doors their wanderguard bracelet alerts staff. The device activity report tracks this as well. For added measure the facility does have a security guard in the front of the building monitoring.

Regarding the allegation of Not enough staff to meet resident needs, the Department found the following; based on interview and record review, it was determined that caregivers respond to call buttons within a reasonable amount of time. LPA interviewed 7 residents who stated they had no complaints regarding staff responding to their call buttons on time. LPA reviewed the device activity report as well and on average caregivers responded within a reasonable amount of time.

Although the allegations may have happened and/or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
