On October 13, 2022 at 9:15AM Licensing Program Analyst (LPA) Chris Hopkins arrived at this facility unannounced to conduct a complaint investigation regarding the above allegations. LPA met with Administrator Mike Talani and explained the purpose of the visit.

Regarding the allegation of Staff did not seek medical attention in a timely manner, the Department found the following; based on interview LPA could not determine whether or not staff called 911 one hour after being told by Resident 1 (R1) family to do so.

Regarding the allegation of Staff did not provide adequate food service, the Department found the following; based on interview, it was determined that some residents were getting their breakfast around 10am due to the facility having multiple Covid-19 outbreaks along with gastrointestinal outbreaks. Staff would prepare found and individually deliver to each room. Although the allegations may have happened and/or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

On October 13, 2022 at 9:15AM Licensing Program Analyst (LPA) Chris Hopkins arrived at this facility unannounced to conduct a complaint investigation regarding the above allegations. LPA met with Administrator Mike Talani and explained the purpose of the visit.

Regarding the allegation of Staff did not administer resident's medications in a timely manner, the Department found the following; based on interview and record review, it was determined that Resident 1 (R1) did have to wait to receive pain medication, a medication that was listed as needed. R1 was on hospice. One staff acknowledged that R1 did wait, so that staff got a doctors order to keep medication in R1's room.

Regarding the allegation of Staff did not respond to resident's call button in a timely manner, the Department found the following; based on interview and record review, it was determined that staff did take a substantial amount of time (at times) in responding to R1's call button.

Regarding the allegation of Resident's laundry needs were not being met, the Department found the following; based on interview, it was determined that R1's laundry was not done often times the days that the facility had laundry scheduled to be done. R1's family reached out to staff multiple times regarding this issue.

Based on interview and record review, the preponderance of evidence standards has been met, therefore, the above allegation(s) is/are found to be SUBSTANTIATED. Per California Code of Regulations, Title 22 Division 6, Chapter 8, deficiencies are being cited on the attached 9099D during this visit. Appeal rights given
