25 | On July 21, 2021 at 2:10pm Licensing Program Analyst (LPA) Chris Hopkins made an unannounced visit to conduct a case management regarding change of ownership notice. LPA met with Executive Director Gregory Greene.
LPA informed Executive Director that CDSS Community Care Licensing never received a Change of Ownership notice from the new ownership company (Atlantis SAC OPCO LLC) taking over. Per the Executive Director residents received a change of management notice but not a change of ownership notice. The change of management notice was provided to LPA.
The following deficiencies were observed and cited Per California Code of Regulations, Title 22 during today's inspection. An exit interview was conducted, a copy of this report and appeal rights were left at the facility. |
