25 | Licensing Program Analyst (LPA) Avelina Martinez arrived at facility unannounced to conduct a case management visit on 10/26/2021. LPA met with Melissa Orello and explained the purpose of the visit.
The purpose of the case management visit is to follow up on a learned deficiency during a complaint investigation. It was learned resident 1 (R1) has an unassessed unstaged pressure injury. LPA Martinez was informed by staff that R1 is not receiving wound care, and R1's pressure injury has not been assessed by a medical professional. R1's first initial pressure injury was reported to Memory Care Director by a caregiver around September 27, 2021.
While LPA Martinez was visiting the facility on 10/26/2021, the Memory Care Director observed R1's pressure injury. It was reported R1 had pink skin and a hole opening on his coccyx area. As of today, the facility has not implemented an assessment plan to meet R1's pressure injury medical needs. Furthermore, R1's pressure injury has not been assessed by a medical professional, and it is unknown what stage the pressure injury is. As a result the facility is not meeting R1's medical needs.
The following deficiency was observed and cited from the California Code of Regulations, Title 22, and California Health and Safety Code.
Exit interview conducted and 809 report and appeal rights given to the facility.
