On 8/23/2021 at 11:25am Licensing Program Analyst (LPA) Chris Hopkins arrived unannounced to deliver complaint investigation findings. LPA met with Administrator Gregory Greene and stated the purpose of the visit. This visit is to deliver the findings of the complaint investigation.

Regarding the allegation Facility does not have adequate staff to meet resident's needs, the Department found the following: based on record review and interview the facility was in the process of hiring staff while letting staff go simultaneously. There is not sufficient evidence to suggest the facility was short staffed. Regarding the allegation Facility's AC is in disrepair, the Department found the following: based on record review and interview the AC was broken and the facility was aware and they have addressed this issue in a timely fashion. As soon as it was broken the facility called to have this fixed. They are in the process of rebuilding the chiller.

As a result of this investigation, this Department finds the allegations to be UNSUBSTANTIATED. A complaint allegation finding of Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.

On 8/23/2021 at 11:25am Licensing Program Analyst (LPA) Chris Hopkins arrived unannounced to deliver complaint investigation findings. LPA met with Administrator Gregory Greene and stated the purpose of the visit. This visit is to deliver the findings of the complaint investigation.

Regarding the allegation of Staff do not respond to resident's call button in a timely manner, the Department found the following: based on record review, there were numerous times of 25 minutes plus on the pendant time log. This log stated the time the pendant was pressed and the time the staff turned the pendant off. Per the Administrator, staff told him they forgot to turn the pendant off when they responded to the resident. Administrator stated that wasn't good enough and the record shows other wise.

Personal assistance and care as needed by the resident and as indicated in the pre-admission appraisal, with those activities of daily living such as dressing, eating, bathing and assistance with taking prescribed medications, as specified in Section 87608, Postural Supports.

Administrator has agreed to implement a new system, to where 1 person (dispatcher) will dispatch caretakers to residents room when pendant is pushed. Administrator will send LPA a written notice of this. POC due by 8/27/2021

This requirement was not met by: Based on record review, the licensee did not ensure residents were seen in a timely manner based on the pendant time log. This poses a potential health and safety risk to residents in care.

Regarding the allegation of Residents did not have access to water, the Department found the following: based on interview and observation, the residents have access to water. There are water stations as well as Administrator has water bottles to give out, anytime the residents need.

The Department has investigated the allegation noted above and have found that this complaint was UNFOUNDED, meaning that the allegation was false, could not have happened and/or was without a reasonable basis. This Department has therefore dismissed the complaint.
