25 | Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct a case management visit to follow up on an incident report. LPA met with facility staff an explained the purpose of the visit.
The facility is level 4I home. LPA received an unusual incident report dated 07/29/23 regarding Resident 1 (R1) and Resident 2 (R2). Based on Individual Program Plan (IPP) dated 05/15/2023, R1 has behaviors that include physical aggression and throwing rocks at windows. R1 had behaviors on 07/29/23 that resulted in R1 throwing items at staff, punching R2 in the face, throwing a rock at the window, and picking up glass from the broken window. Staff on shift attempted to redirect resident each time a behavior occurred but was unsuccessful. Staff contacted 911 due to a danger to self and others. R1 was taken to the hospital for evaluation. First-aid was not required for R2. R1 returned on August 9th, 2023. Administrator informed Alta Regional Center about the medication changes for R1. Since the medication changes, R1 has not shown aggressive physical behaviors towards residents.
LPA observed the window that was broken. The window is located in the front of the home. There are no sharp points that are accessible to residents. According to facility designated administrator, Orlando the window repair person stated the window will be replaced in 3-4 weeks due to the size of the window.
LPA observed 3 staff on shift and 4 residents in care. Residents were observed taking a nap, conducting activities with staff, and playing on the electronic devices. No health or safety concerns observed.
Per California Code of Regulations (CCR) - Title 22, no deficiencies are being cited. An exit interview held, and a copy of the report was provided. |
