32 | Interview with facility staff revealed that, no residents have expressed concerns with being harassed by other residents. Facility staff stated, that occasional redirects of residents are sometimes needed, and facility staff closely monitors residents in care. Additionally, interview with an outside agency revealed that resident (R2) did not express any concern with being harassed by other residents in care. During observation of the facility, it was noted that residents in care were all in their own rooms and common areas engaging in ADL’s, no inappropriate interactions were observed. Staff were present and appeared to be monitoring resident interactions appropriately. Based on the information obtained, there is no evidence to support the allegation, therefore it could not be corroborated at this time.
Allegation: Staff allows a resident to barricade themselves while in care
It was alleged that staff allows a resident to barricade themselves while in care. This investigation consisted of interview with residents and staff, and facility observation. On 6/17/2025 LPA Shakaricka Hughes conducted an unannounced facility visit, during the visit LPA Hughes interviewed 4 out of 5 residents who did not express any concerns about the need to barricade themselves in their room. Interview with facility staff revealed that, they have not observed residents barricading themselves in their rooms, and that residents are able to close their doors whenever they feel it is appropriate. During a tour of the facility, LPA Hughes observed residents door rooms 1, 2, 3, 5 without locks attached, no chairs or other objects blocking the door were observed in the resident’s room at the time of the visit, and there was no evidence to support the allegation that the resident’s door(s) had been barricaded. Based on the information obtained, there is no evidence to support the allegation, therefore it could not be corroborated at this time.
The investigation revealed the preponderance of evidence standards have not been met; therefore, the above allegations are found to be UNSUBSTANTIATED. A finding that the complaint allegations are UNSUBSTANTIATED means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.
