25 | Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to continue the investigation into the open complaint. LPA met with Kathleen Gilby, Administrator, and explained purpose of inspection. Prior to initiating today's inspection, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms;. LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N95 mask. Additionally, LPA was screened upon entering the facility.
While at the facility, LPA and Administrator discussed current Covid situation- last positive case (staff) has completed 10-day quarantine. Second round of response testing was conducted yesterday.
Also discussed during today's inspection is the death of resident (R1) last Saturday, 3/5/2022. Facility submitted a completed incident report on Monday, 3/7/22. Resident was not receiving hospice services but had moved in recently to the facility. Administrator agreed to reach out to resident's family and request a copy of the county death certificate and provide to the Department when received.
There are no deficiencies being issued today.
