25 | On July 21, 2026, at 9:30 AM, Licensing Program Analysts (LPAs) Avelina Martinez and Sulma Lopez made an unannounced visit to this facility to conduct an annual required inspection. LPAs met with Raymond Vizcarra and explained the purpose of today's visit.
LPAs inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, activity room, and outside courtyards of the facility to ensure compliance with Title 22 regulations.
Administrator holds current certificate. The facility is licensed for six non-ambulatory residents. There are currently five residents who reside at this facility. The facility has an approved hospice waiver for three.
The LPA Martinez toured the facility with Raymond Vizcarra on July 21, 2026, at 10:15 AM.
LPA Martinez reviewed two resident files and two staff files, which were maintained. LPA Lopez reviewed two medication administration records (MAR) were maintained. The facility has an infection control plan and a disaster plan. The last emergency drill was on July 15, 2026. The carbon detectors, smoke detectors, and fire extinguishers were in good repair. The facility had an adequate food supply. The facility bedroom, common areas, and bathrooms were furnished and in good repair. The exterior of the home is clear of debris, and the emergency exit gate is in good repair. The exterior patio is furnished and in good repair.
Based on this annual inspection there were no deficiencies cited. An exit interview was conducted, and a copy of this report was provided to the facility.
