25 | On 5/20/22, Licensing Program Analyst (LPA) Jacob Williams, conducted an unannounced case management visit. Prior to initiating the complaint visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms. Upon arrival, completed a facility risk assessment. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: Surgical Mask.
The purpose of this inspection was to ensure facility is keeping up with compliance measures.
While at the facility for a the case management issue, Licensing Program Analyst (LPA) Jacob Williams conducted this inspection to follow-up on legal/non-compliance actions. LPA met with Licensee Vera Okyere and discussed the purpose of the inspection.
R1 had two medications not present. R2 had three PRN medications that did not have a doctors order.
Deficiencies cited on LIC809-D attached. Civil Penalties have been issued. Appeal rights provided.
