25 | On October 24, 2024, Licensing Program Analyst (LPA) Loraine Perez and Lea Habtom arrived at Sunrise Kids Club at the Oak Hills Elementary School Campus for an unannounced case management inspection. At the time of arrival, LPAs met with Site Director, Felicia Martell and Courtney Hobart, Executive Director. The purpose of this case management is to address the staff that did not have fingerprint clearance/transfer prior to working at the facility. On September 12, 2024, LPAs opened a complaint and gathered the names of the staff present providing care and supervision. There were 9 staff present on September 12, 2024, and 1 staff was not fingerprint cleared. LPAs reminded Director that all staff must obtain a criminal record clearance, exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Child Care Center. LPAs record reviews and interviews provided evidence that one staff worked in the facility on September 12, 2024, without having their clearance or documentation transferred to the facility.
Based on today's inspection, one Type A deficiency is being cited on 809-D. Exit interview was conducted and this report was reviewed with Director, Felicia Martell, has been provided with appeal rights and this report. Notice of Site was provided and must remain posted for 30 days.
Title 22 Deficiency has been cited on the attached LIC 809-D. LPA Lea Habtom informed Director Felicia Martell that this report dated October 24, 2024, document(s) 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the Director to provide a copy of this licensing report dated October 24, 2024, that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal Rights given.
