On 05/14/24, Licensing Program Analyst (LPA) Jennie Tedlos arrived at the facility for the purpose conducting a plan of correction inspection. LPA met with Facility Representative, Perry Leonard. LPA observed 10 children supervised by three staff.

The facility was inspected on 03/20/24 at which time a deficiency was cited regarding personnel files. A plan of correction was developed during the inspection. LPA reviewed personnel files during today's inspection and observed the required documents.

The deficiency cited on 03/20/24 is cleared effective today. A proof of correction letter was provided for the corrected deficiencies. LPA reviewed report with the Director, Perry Leonard. Appeal Rights were provided. A notice of site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
