13 | On 6/18/26 Licensing Program Analyst Denice Pablico (LPA1) and Katy Valasquez (LPA 2) conducted an unannounced complaint investigation regarding the allegation listed above. LPAs arrived at the Center and observed 2 preschool aged children being supervised by 1 staff member. LPAs accessed Guardian to determine that all required adults were background cleared and associated with the license. LPAs met with Director Perry Leonard (D1). Throughout the course of the investigation, LPAs conducted physical plant inspections, on-site observations, interviews, reviewed and collected documentation. It was alleged that staff do not ensure that facility is clean.
It was alleged that the facility is unclean. LPAs inspected the inside and outside of the facility and did not observe any uncleanliness. LPAs inspected the kitchen and food storage. L1 stated that food is not provided by the facility but by the parent(s) of the enrolled children. LPAs interviewed child(ren), and reviewed documentation. Based on observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED.
