Licensing Program Analysts Erwina Pascual-Golamco and Jennifer Velasco (LPAs) conducted unannounced complaint investigation inspection and met with Director Kasey Yarbrough (D1). LPA toured the infant facility, including all activity and classroom spaces, restrooms, food service and outdoor play areas. Census included six infant children being supervised by two classroom staff. D1 was reminded never to exceed the conditions, limitations, and capacity specified on the license. Facility hours of operation are Monday through Friday from 7:30 AM to 5:30 PM.
It was alleged the facility is operating out of ratio. It was also alleged staff did not provide a safe and comfortable environment for daycare children. During the investigation, LPA conducted interviews, observed staff provide care to children, and requested relevant documentation.
It was alleged the facility operated out of ratio. Ratio for awake infant care is one qualified staff caring for four awake infants (children under two years of age). Witness statements, LPA observations, and facility documentation failed to corroborate the allegation that the facility operated out of ratio.

It was also alleged staff did not provide a safe and comfortable environment for infant children in care, specifically, that staff left a sleeping infant on the floor in an indoor and/or outdoor awake activity area or in a wagon or other item not compliant with safe sleep regulations. Witness statements, LPA observation, and facility documentation failed to corroborate the allegation that the staff did not comply with requirements to provide a safe and comfortable environment for infant children in care. The preponderance of evidence standard has not been met; therefore, the allegations are unsubstantiated.

An exit interview was conducted, and a notice of site visit provided. Notice of site visit shall remain posted for 30 days.
