On October 12, 2022, at approximately 08:15 AM, Licensing Program Analyst (LPA ) Alize Tillery met with Director Kasey Yarbrough for the purpose of an unannounced required 1 year annual inspection. Kasey Yarbrough is new to the director position, as of 2/17/2022. Director was not associated to the facility and had expired fingerprints on file; Director will be refingerprinted. Upon arrival, LPA observed 9 infant children, supervised by 5 staff. The facility hours of operation are Monday through Friday from 7:30 AM to 5:30 PM. The facility does not offer transportation services.

LPA toured all activity and classroom spaces (Infant Room), the kitchen area, the napping area and the outdoor play area. LPA observed the following documents are posted: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, menus, and daily schedule. Cleaning disinfectants, medications and hazardous items are appropriately stored and inaccessible to children. Director stated there are no poisons, weapons or bodies of water on the premises. Furniture and equipment are in good condition, and toileting facilities are in safe, sanitary and operating condition. LPA observed one changing table in arms reach of a sink. LPA observed infant food and bottles labeled in the refrigerator. The floors appeared clean throughout the facility. The food preparation space is free of litter, all food was protected against contamination, and storage containers with solid waste have covers.



Drinking water is readily available to children both indoors and outdoors. LPA observed a water pitcher and children’s personal cups. LPA observed the electronic sign in and out system. Playground equipment and surfaces are free of loose or sharp parts. There is rubber cushion beneath the playground equipment in the outdoor play area. There are sufficient equipment and toys. There are shaded areas supplied by a large tent and building.

4 staff files and 5 present day care children file records were reviewed. All children and staff files were observed to be complete. Program is documenting the napping infants, every 15 minutes, LPA observed cribs with a empty crib with a mattress and tight fitted sheets. Director stated that infant sheets are changed daily. LPA observed children’s individual sleep plan and needs and services plan. At least one staff member present today has current Pediatric CPR and First Aid certification. LPA observed a smoke and carbon monoxide detector. LPA reviewed the Department's inspection authority and discussed with designee any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within ten working days.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Deficiencies are cited on the following 809D page. A notice of site visit was given and must remain posted for 30 days.
