On 03/05/2026, Licensing Program Analyst Katy Velazquez (LPA) conducted an unannounced complaint investigation. LPA arrived at the Center and observed 54 preschool aged children being supervised by 5 staff members. LPA accessed Guardian to determine that all required adults were background cleared and associated to the license.LPA met with Director Alyssa DiPippo (D1).
Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, interviews, reviewed and collected documentation. It was alleged that staff members did not prevent children in care from engaging in harmful behaviors that resulted in injuries. LPA reviewed incident reports that document 8 bites and 16 incidents of hitting/scratching/pinching in the Discovery Preschool classroom for the months of January and February 2026. Interviews with staff members revealed that the combination of early 2 year olds and 3 year olds had presented supervision issues that contributed to not being able to prevent the harmful behaviors that led to injuries. LPA observed and inspected the Discovery classroom.

On 03/05/2026, Licensing Program Analyst Katy Velazquez (LPA) conducted an unannounced complaint investigation. LPA arrived at the Center and observed 54 preschool aged children being supervised by 5 staff members. LPA accessed Guardian to determine that all required adults were background cleared and associated to the license. LPA met with Director Alyssa DiPippo (D1).
Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, interviews, reviewed and collected documentation. It was alleged that staff members refused to assist children with their toileting needs. Interviews with staff members revealed that 3 year old children were not moved up to the 3 year old classroom because they were not yet fully potty trained. Administration made the decision to not transition these children, not due to staff refusing to potty train, but due to lack of staff members in the 3 year old classroom for proper potty training supervision. The facility was in the process of interviewing and hiring to resolve this staffing issue.

Based on interviews, observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED. In the areas that were evaluated on 03/05/2026, no deficiencies were cited during today's inspection. An exit interview was conducted with Director DiPippo and Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

LPA observed food and milk on the floor and tables, every book from the classroom library discarded on the floor, and children throwing bean bags at one another.
Based on interviews, file reviews, and observations conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A Type-A deficiency was cited on a subsequent 9099-D page. D1 acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 9099-D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. An exit interview was conducted, and the report was reviewed with Director DiPippo. LPA provided Licensee Appeal Rights to D1. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
