Licensing Program Analyst (LPA) Jennifer Velasco met with Facility Representative, Director Carrie Robinson (Director), to conduct an unannounced complaint investigation inspection pertaining to the above allegations. The purpose of today's inspection was explained. During the investigation, LPA conducted interviews, observed care, and requested relevant documents. Witness statements, LPA observations, andr document reviews failed to corroborate the allegations.

It was alleged staff used an inappropraite form of punishment with a child, namely, taking away the child's shoes. Multiple witnesses stated the child took off their own shoes and refused to wear them until they were picked up.

It was alleged staff handled a child in a rough manner. Multiple witneses stated staff did not handle a child roughly but used redirection and verbal instruciton to manage behavior.

It was alleged staff pinched a child. Multiple witnesses stated some children did pinch teachers and each other, but staff did not pinch children and did create and implement a plan to prevent children pinching each other and teachers.

It was alleged staff caused injury to s child, specifically, that a teacher pushed a child's forehead and caused a bump on it. Multiple witnesses stated teachers do not use physical punishment or cause injury to children.

It was alleged the facility is dirty. Multiple witnesses stated that, though there can be messy moments during meals or art projects, the facility is kept as clean and tidy as possible throughout the day.

It was alleged the facility smells bad. Multiple witnesses stated there can be times when a staff is changing a diaper that there is odor from the dirty diaper, but that the dirty diapers are immediately bagged and placed in closed trash cans, and that the trash cans are emptied frequently throughout the day.

Although the allegations may have happened, there is not a preponderance of evidence to prove the allegations; therefore, the allegations are unsubstantiated. Exit interview was conducted and report was reviewed with Facility Representative, Carrie Robinson. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
